Important screenings for LGBTQ family planning

If you’re planning to or may need to use a surrogate in the future, there are additional screenings necessary for you before you freeze your sperm, including an STI test. This is required by the FDA in order to use your sample with a surrogate in the future. There are no pre-screenings required for an analysis.

FAQs abxout LGBTQ fertility options

Pregnancy options for gay couples vary depending on the biology of each member of the partnership.

  • For gay couples with two cisgender men or any two people without uteruses, gestational surrogacy is an option that can allow them to have biological children.
  • For lesbian couples with two cisgender women or any two people with uteruses, sperm donation from a known or anonymous donor is an option for building a family.

We know the process of building a family can feel complicated. We’re here to make it easier and more affordable.

If you are planning to build your family through thirdparty reproduction — AKA, you might use a surrogate or sperm donor — there are some additional screenings that the sperm-producing person should undergo before freezing their sample. These screenings are required by the FDA.

Screenings include, but are not limited to:

  • A physical exam
  • Infectious disease testing with an FDA-cleared lab:
  • Gonorrhea and chlamydia urine cultures
  • HIV-I and HIV-II screening
  • Hepatitis B surface antigen and Hepatitis B core antibody
  • Hepatitis C antibody
  • Syphilis RPR
  • CMV
  • HTLV-I and HTLV-II

These screenings apply only to those working with a surrogate in the US. If you’re working with a surrogate abroad, talk with your surrogacy agency or fertility clinic to understand any screening requirements.

Short answer: as much as you can afford. A few questions you might consider:

  • Do you want one child, multiple children, or maybe you’re not sure you want kids at all? How much sperm you should freeze will change depending on family size and how certain you are that you want biological children in the future.
  • Will you be using IUI or IVF to attempt pregnancy? Generally speaking, one vial of sperm is used in each IUI or IVF procedure, and 1 semen sample = 4 vials of frozen sperm. IUI pregnancy rates range from 10–25% per cycle; IVF success rates range from 5–60% per cycle. So, you may need multiple attempts to achieve pregnancy.

As long as sperm is kept frozen in a high quality cryogenic lab like Legacy’s, it can be stored indefinitely with no significant impact on its quality or the chances that it will result in a healthy pregnancy. That means you can keep your sperm frozen until you’re ready to start a family.

The chances of conceiving with frozen sperm depend on several factors:

  • The quality and quantity of the sperm
  • The quality of the donor egg (if using a surrogate) or the age and overall fertility health of the person carrying the pregnancy
  • Whether you use intrauterine insemination (IUI), in vitro fertilization (IVF), or intracytoplasmic sperm injection (ICSI)

IUI pregnancy rates range from 10–25% per cycle; IVF success rates range from 5–60% per cycle. So, you may need multiple attempts to achieve pregnancy. The good news is that using frozen sperm doesn’t affect your chances of success. IUI and IVF pregnancy rates are the same for those using frozen sperm, compared to “fresh.”
