Let\u2019s take a look at this common issue and what it could mean for your fertility.<\/span><\/p>\n<p><em>Pronunciation: <strong>ver<\/strong>-a-ko-seal. (If you get it wrong, don&#8217;t worry\u2014you&#8217;re not alone.)<\/em><\/p>\n<h2><span style=\"font-weight: 400;\">What is a varicocele?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Varicocele is an enlargement of the veins within the scrotum, the loose skin that houses the testicles. It\u2019s similar to varicose veins in the legs, and it\u2019s relatively common, occurring in roughly one in six (over 15%) people assigned male at birth.<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">What causes varicocele?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Researchers believe that varicocele is caused by defects in the valve(s) that regulate the blood flow to and from the testicles. When the valves have a fault, blood can back up within the veins, causing bulging and stretching.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In most cases, this happens during puberty, when changes to the sex organs increase blood flow. Interestingly enough, it happens most often\u2014around 90% of the time\u2014in the left testicle, but can also occur in the right testicle or in both (known as bilateral varicocele).<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">What are the symptoms of varicocele?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Varicocele doesn\u2019t usually cause any symptoms, and most people with this condition don\u2019t even notice. If there are symptoms, <\/span><a href=\"https:\/\/www.mayoclinic.org\/diseases-conditions\/varicocele\/symptoms-causes\/syc-20378771#:~:text=A%20varicocele%20\"><span style=\"font-weight: 400;\">they often manifest as<\/span><\/a><span style=\"font-weight: 400;\">:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">A periodic ache or heaviness in the testicles that worsens as the person goes through the day.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Increase in discomfort during hot temperatures or weather.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Pain or pressure after doing something physical.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Relief when lying down.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Fertility issues.<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">Having varicocele doesn\u2019t <\/span><i><span style=\"font-weight: 400;\">automatically<\/span><\/i><span style=\"font-weight: 400;\"> mean it\u2019s affecting sperm health. However, varicocele is associated with infertility; <\/span><a href=\"https:\/\/www.uclahealth.org\/urology\/body.cfm?id=478&amp;action=detail&amp;ref=19\"><span style=\"font-weight: 400;\">according to UCLA Health<\/span><\/a><span style=\"font-weight: 400;\">, about 40 percent of males experiencing infertility also had varicocele in at least one testicle.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Varicocele can also cause the affected side to shrink. Medical professionals aren\u2019t entirely sure of the relationship between damaged veins and shrinkage of the testicle, but it could be due to increased pressure or exposure to toxins in the blood.<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">How do medical professionals diagnose varicocele?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Since most people with testicles may not even realize they have varicocele, diagnosis happens through a physical exam. A doctor examining the testicle could notice visible veins and confirm through a small physical exam.&nbsp;<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Smaller varicoceles can be diagnosed with Doppler ultrasonography, a technique that listens for backflow through the valves. Doctors may also use thermography, an infrared process that senses pockets of heat created by the pools of blood in the enlarged veins.<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">Varicocele and male fertility<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">There has been ongoing debate about the effects of varicocele on fertility, but the most recent literature strongly suggests a link. A <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3046876\/\"><span style=\"font-weight: 400;\">massive literature review<\/span><\/a><span style=\"font-weight: 400;\"> published in 2011 found that, despite conflicting studies, some research found significant improvements in fertility after varicocele repair.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Varicocele doesn\u2019t cause blanket infertility, but it may be associated with altered semen parameters. Why? Some potential mechanisms:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Increased heat.<\/b><span style=\"font-weight: 400;\"> The ideal temperatures in the testicles is about 5\u00baF below normal body temp. The blood pooling in the veins, known as venous reflux, may raise temperatures in the testicles just enough to affect sperm production.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Oxidative stress.<\/b><span style=\"font-weight: 400;\"> Reactive oxygen species (ROS), or \u201cfree radicals,\u201d are natural byproducts of cellular metabolism; their levels are elevated as a result of infection, inflammation, toxin exposure, etc. When ROS levels get too high, they can damage cells\u2014including sperm cells. This is known as oxidative stress. <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/16790111\/\"><span style=\"font-weight: 400;\">Studies<\/span><\/a><span style=\"font-weight: 400;\"> demonstrate that men with varicocele(s) have higher levels of reactive oxygen species in their semen.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Hormone imbalance.<\/b><span style=\"font-weight: 400;\"> Because testosterone is produced in the testicles, varicocele may affect lower testosterone levels and affect the balance of <\/span><a href=\"https:\/\/givelegacy.com\/resources\/the-hormones-that-drive-male-fertility\/\"><span style=\"font-weight: 400;\">male fertility hormones<\/span><\/a><b>. <\/b><span style=\"font-weight: 400;\">Changes in hormones can affect overall sperm quality.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Breach in the blood-tissue barrier in the testis.<\/b><span style=\"font-weight: 400;\"> The barrier between the blood and the tissue of the testicle typically protects sperm from any immune cells that may otherwise attack them. However, <\/span><a href=\"https:\/\/www.sciencedirect.com\/topics\/biochemistry-genetics-and-molecular-biology\/blood-testis-barrier\"><span style=\"font-weight: 400;\">studies<\/span><\/a><span style=\"font-weight: 400;\"> show a correlation between the presence of varicocele and anti-sperm antibodies (immune cells that destroy sperm), suggesting varicocele may cause a breach in the BTB.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Reduced blood flow to the testes.<\/b><span style=\"font-weight: 400;\"> Altering the amount of blood the testes receive could affect the environment, production, and <a href=\"https:\/\/www.givelegacy.com\/sperm-quality\/\">quality of sperm<\/a>.<\/span><\/li>\n<\/ul>\n<h2><span style=\"font-weight: 400;\">Impact of varicocele on male fertility parameters<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">According to a <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/33689198\/\"><span style=\"font-weight: 400;\">study<\/span><\/a><span style=\"font-weight: 400;\"> from March of this year, varicocele patients have a significant reduction in average semen parameters and an increase in <\/span><a href=\"https:\/\/givelegacy.com\/sperm-dna-fragmentation\"><span style=\"font-weight: 400;\">sperm DNA fragmentation<\/span><\/a><span style=\"font-weight: 400;\">, damage to the genetic material carried by sperm. The study examined 84 varicocele patients and 85 healthy patients. Researchers suspected that chronic inflammation caused by varicocele, and the associated oxidative stress, reduced overall semen parameters.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Additionally studies provide further evidence connecting varicocele and sperm DNA fragmentation. <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/16500329\/\"><span style=\"font-weight: 400;\">A 2006 study<\/span><\/a><span style=\"font-weight: 400;\"> found a higher percentage of class III and class IV DNA fragmentation (meaning, high levels of damage) in adolescents with at least one varicocele-affected testicle. <\/span><a href=\"https:\/\/www.notion.so\/Varicocele-and-male-fertility-d604ce69670a43c9b28618df5ba617d3\"><span style=\"font-weight: 400;\">Another study<\/span><\/a><span style=\"font-weight: 400;\"> pointed to chronic inflammation as a potential source of <a href=\"https:\/\/www.givelegacy.com\/what-is-sperm\/\">sperm<\/a> disruption.<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">Should I treat my varicocele? What are the treatment options for varicocele?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Varicocele doesn\u2019t always require treatment. If there\u2019s no ongoing pain or discomfort, or no fertility issues, treatment\u2014especially surgery\u2014isn\u2019t an automatic path. However, for those experiencing either challenge, there are surgical and nonsurgical treatment options.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">If it\u2019s causing pain or discomfort, treatment can reduce those symptoms. For those of you considering future family planning, it can help to make an informed decision about seeking treatment.<\/span><\/p>\n<ol>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Test your sperm:<\/b><span style=\"font-weight: 400;\"> The first thing to do is complete an <\/span><a href=\"https:\/\/givelegacy.com\/sperm-testing-guide\/\"><span style=\"font-weight: 400;\">at-home sperm testing kit<\/span><\/a><span style=\"font-weight: 400;\"> along with a <\/span><a href=\"https:\/\/givelegacy.com\/sperm-dna-fragmentation-analysis\/\"><span style=\"font-weight: 400;\">sperm DNA fragmentation analysis<\/span><\/a><span style=\"font-weight: 400;\"> to give you a complete picture of your sperm health. If your varicocele isn\u2019t affecting your semen parameters or sperm genetic health\u2014and it\u2019s not causing you pain\u2014you may not need to seek treatment at all.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Talk to your medical professional:<\/b><span style=\"font-weight: 400;\"> If your <a href=\"https:\/\/www.givelegacy.com\/sperm-testing-guide\/\">semen analysis<\/a> comes back with abnormalities, you can discuss the right treatment path with your medical professional.<\/span><\/li>\n<\/ol>\n<h3><span style=\"font-weight: 400;\">Nonsurgical treatment for varicocele<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Treatment can be as simple as anti-inflammatory medications, or wearing snug underwear or a jockstrap to provide support for the testicles. These treatments reduce discomfort, but may not reverse any effect of the varicocele on male fertility.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Embolization is also a nonsurgical treatment option. During embolization, a catheter is used to place a coil or other obstacle into the damaged vein to block it and redirect blood flow. Embolization is less invasive than surgery, and is an outpatient procedure with a shorter healing period. However, embolization may have <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3433543\/\"><span style=\"font-weight: 400;\">a higher rate of failure<\/span><\/a><span style=\"font-weight: 400;\"> than surgical varicocele repair.<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">Surgical repair for varicocele<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Similar to embolization, surgery can help redirect the flow of blood to reduce pockets and fix any valve issues causing the flow issues in the first place. The surgeon ties off the unhealthy vein while preserving the overall vein and artery structure. This process is known as \u201cvaricocelectomy.\u201d<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">Post-treatment improvement in semen parameters<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Studies show a strong link between varicocele repair surgery and both <a href=\"https:\/\/www.givelegacy.com\/sperm-improvement-guide\/\">improved sperm parameters<\/a> and increased rates of pregnancy. Some <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3433543\/\"><span style=\"font-weight: 400;\">studies<\/span><\/a><span style=\"font-weight: 400;\"> also estimate that embolization, if successful, will produce a similar improvement.<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/27526630\/\"><span style=\"font-weight: 400;\">A 2016 study<\/span><\/a><span style=\"font-weight: 400;\"> found a positive correlation between reconstructive surgery and live birth rates. This meta-analysis looked at 7 studies totaling 1,241 azoospermic (zero sperm count) and oligospermic (low sperm count) males with varicoceles.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/33763931\/\"><span style=\"font-weight: 400;\">A 2021 study<\/span><\/a><span style=\"font-weight: 400;\"> found a statistically significant increase in motile sperm count (from less than 5 million before surgery to between 5 and 9 million after), as well as increased sperm quality post-surgical repair.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">A small <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/29051002\/\"><span style=\"font-weight: 400;\">2018 study<\/span><\/a><span style=\"font-weight: 400;\"> looked at 17 patients who had varicocele surgery. After reparative surgery, sperm concentration increased by 4.3 million sperm\/mL and motility improved by 12%.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/33355829\/\"><span style=\"font-weight: 400;\">A 2020 study<\/span><\/a><span style=\"font-weight: 400;\"> looked at patients that underwent surgical correction versus patients who opted for <a href=\"https:\/\/www.givelegacy.com\/future-fertility-treatments\/\">fertility treatment<\/a> without varicocelectomy. The study found an improvement in pregnancy rates in the group that underwent varicocele repair.<\/span><\/li>\n<\/ul>\n<h2><span style=\"font-weight: 400;\">Living with varicocele<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">If you find out you have varicocele, the first step should be to get a deeper picture of your sperm health through <\/span><a href=\"https:\/\/givelegacy.com\/sperm-analysis\/\"><span style=\"font-weight: 400;\">at-home semen analysis<\/span><\/a><span style=\"font-weight: 400;\">.
