Free radicals are produced by a number of natural body processes, including metabolism, and can be increased by exposure to radiation or pollution, smoking, or illness.<\/p>\n\n\n\n<p>Free radicals can cause damage by \u201cstealing\u201d electrons from other cells. This damage can contribute to many diseases.<\/p>\n\n\n\n<div style=\"height:20px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h3 class=\"wp-block-heading\">Reactive oxygen species (ROS)<\/h3>\n\n\n\n<p>ROS are a type of free radical that contains oxygen. Like other free radicals, they can cause cell injury.<\/p>\n\n\n\n<div style=\"height:20px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h3 class=\"wp-block-heading\">Antioxidants<\/h3>\n\n\n\n<p>Antioxidants are nutrients that prevent or slow cellular damage caused by free radicals. Antioxidants lend an electron to free radicals, thereby neutralizing and stabilizing them. Examples of antioxidant nutrients include vitamin C, vitamin E, and CoQ10.<\/p>\n\n\n\n<div style=\"height:50px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h2 class=\"wp-block-heading\">What is oxidative stress?<\/h2>\n\n\n\n<p>Free radicals are inevitable, since they\u2019re a natural byproduct of human life. Ideally, free radicals can be neutralized by antioxidants before they can cause real damage to your cells.<\/p>\n\n\n\n<p>When there are too many free radicals in your body and not enough antioxidants, the free radicals can begin to damage your DNA, proteins, and lipids (fatty tissue). This is oxidative stress.<\/p>\n\n\n\n<p>Scientists define <a href=\"https:\/\/www.sciencedirect.com\/topics\/neuroscience\/oxidative-stress\">oxidative stress<\/a> as &#8220;an excess production of reactive oxygen species (ROS) relative to antioxidant defense.&#8221; Essentially, this means your body has an imbalance between free radicals and antioxidants. In other words, the ratio of free radicals is too high, and the antioxidants can\u2019t keep up.<\/p>\n\n\n\n<p>Reactive oxygen species aren\u2019t all bad. Free radicals can actually help fight off pathogens that may lead to infection. However, the issue comes when there is long-term exposure to oxidative stress. This can lead to problems such as:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>diabetes<\/li>\n\n\n\n<li>cardiovascular problems like heart disease, hardening of the blood vessels, and high blood pressure<\/li>\n\n\n\n<li>inflammatory conditions<\/li>\n\n\n\n<li>Parkinson&#8217;s disease<\/li>\n\n\n\n<li>Alzheimer&#8217;s disease<\/li>\n\n\n\n<li>various types of cancer<\/li>\n\n\n\n<li>fertility issues<\/li>\n<\/ul>\n\n\n\n<p>Oxidative stress also contributes to signs of aging.<\/p>\n\n\n\n<div style=\"height:50px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h2 class=\"wp-block-heading\">How does oxidative stress affect male fertility?<\/h2>\n\n\n\n<p>One of the cells that oxidative stress can damage: Sperm cells.<\/p>\n\n\n\n<p>Research tells us that <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC5004233\/\">oxidative stress<\/a> significantly impacts male fertility and has harmful effects on sperm, including DNA damage.<\/p>\n\n\n\n<p>Reactive oxygen species can negatively impact sperm cell membranes, which contain a high concentration of unsaturated fatty acids. ROS can cause a process called \u201c<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC6472207\/#:~:text=Current%20evidence%20links%20oxidative%20stress,recurrent%20abortions%20and%20genetic%20diseases.\">peroxidation of lipids<\/a>,\u201d in which free radicals steal electrons from the lipids in cell membranes, leading to cell damage.<\/p>\n\n\n\n<div style=\"height:50px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h2 class=\"wp-block-heading\">Signs of oxidative stress in semen<\/h2>\n\n\n\n<p>Common indicators of oxidative stress on a <a href=\"https:\/\/www.givelegacy.com\/sperm-analysis\/\">semen analysis<\/a> include:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Poor sperm motility (asthenozoospermia)<\/li>\n\n\n\n<li>Poor sperm morphology (teratozoospermia)<\/li>\n\n\n\n<li>High number of round cells, including leukocytes (white blood cells), which could be a sign of infection<\/li>\n\n\n\n<li>Increased sperm DNA damage on a <a href=\"https:\/\/www.givelegacy.com\/sperm-dna-fragmentation-analysis\/\">DNA fragmentation analysis<\/a><\/li>\n<\/ul>\n\n\n\n<p>However, a number of other issues could also be contributing to these results. What are the <em>specific<\/em> signs of oxidative stress we can test in the semen?<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>ROS levels in the semen.<\/strong> <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4429439\/\">Specialized lab tests<\/a> can identify the quantity of reactive oxygen species in the semen. However, further research is still necessary to understand what \u201cnormal\u201d ROS levels look like.<\/li>\n\n\n\n<li><strong>Total antioxidant capacity (TAC) in the semen<\/strong>. Measuring the levels of <a href=\"https:\/\/onlinelibrary.wiley.com\/doi\/abs\/10.1111\/and.13624\">certain antioxidants in the semen<\/a> can give an idea of how well the body is protected against oxidative stress, especially when weighed alongside ROS levels.<\/li>\n\n\n\n<li><strong>MDA levels in the semen.<\/strong> MDA, or malondialdehyde, is an organic compound that\u2019s considered a biological marker for oxidative stress. Specialized tests can detect MDA levels in the semen, which are typically <a href=\"https:\/\/academic.oup.com\/humupd\/article\/14\/3\/243\/683505\">elevated<\/a> in infertile people with excess ROS production.<\/li>\n<\/ul>\n\n\n\n<p>Lower measures of TAC and higher levels of ROS and MDA in the semen have consistently been correlated with poorer semen parameters and fertility outcomes.<\/p>\n\n\n\n<p>For example, <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3929810\/\">one study<\/a> of 42 semen samples found that, on average, samples with low sperm motility, count, and morphology had 40% lower measures of total antioxidant capacity, compared to healthy samples. The MDA levels in the unhealthy samples, on the other hand, were about 56% higher than the levels in healthy samples. Several other small studies have made similar observations.<\/p>\n\n\n\n<p>Oxidative stress plays an <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC6704307\/\">independent role<\/a> in the cause of male infertility, with 30% to 80% of infertile men showing elevated ROS levels in their semen. However, we\u2019re not yet at the point where ROS, TAC, or MDA are common tests for male fertility. While many experts are pushing for these tests to become more mainstream, there is still work to be done to establish a reference range for normal measures.<\/p>\n\n\n\n<p>For now, many doctors may use semen analysis and DNA fragmentation results along with lifestyle, exposure, and medical history to assess whether oxidative stress may be affecting male fertility. <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC5004233\/\">Reducing oxidative stress<\/a> can be a useful treatment strategy for male infertility.<\/p>\n\n\n\n<div style=\"height:50px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h2 class=\"wp-block-heading\">What causes oxidative stress?<\/h2>\n\n\n\n<p>Oxidative stress is caused by the imbalance between the reactive oxygen species (ROS) and antioxidants in the semen. &#8220;Oxidative stress can result from either exogenous or endogenous factors that will eventually put pressure on the sperm quality and quantity,&#8221; explains Dr. Ramy Abou Ghayda, fellowship-trained urologist and chief medical officer at <a href=\"https:\/\/www.givelegacy.com\/\">Legacy<\/a>.&nbsp;<\/p>\n\n\n\n<p>\u201cEndogenous\u201d means internal factors, like a medical condition, such as varicocele, or hormonal abnormality. \u201cExogenous\u201d means external factors, such as lifestyle or environmental exposure like smoking or pollution.<\/p>\n\n\n\n<p>According to Dr. Ghayda, both of these factors can cause an increase in oxidative stress. &#8220;This will eventually and subsequently lead to lower semen parameters and might be associated with an abnormal success rate for fertility, whether through natural or assisted reproductive techniques,&#8221; he discusses.<\/p>\n\n\n\n<p>Exogenous causes are a little more within our control, and include:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>poor diet and obesity<\/li>\n\n\n\n<li>smoking<\/li>\n\n\n\n<li>drinking alcohol<\/li>\n\n\n\n<li>taking certain medicines<\/li>\n\n\n\n<li>exposure to environmental factors such as radiation, toxins, air pollution, and pesticides<\/li>\n<\/ul>\n\n\n\n<div style=\"height:50px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h2 class=\"wp-block-heading\">How to reduce oxidative stress<\/h2>\n\n\n\n<p>Dr. Ghayda discusses there are three ways to help reduce oxidative stress:<\/p>\n\n\n\n<ol class=\"wp-block-list\">\n<li><strong>Try to avoid exposure to exogenous factors.<\/strong>\n<ul class=\"wp-block-list\">\n<li>If you&#8217;re smoking, engage in a smoking cessation program and try to quit.<\/li>\n\n\n\n<li>Consider wearing a mask in areas with high levels of air pollution.<\/li>\n\n\n\n<li>Improve your diet. Include a lot of greens, avoid added sugars and trans fat, and reduce the number of processed foods you eat. Try the Mediterranean diet.<\/li>\n\n\n\n<li>Wear sunscreen.<\/li>\n\n\n\n<li>Make sure to speak with your healthcare provider to understand if any medications you\u2019re taking might negatively impact fertility.<\/li>\n\n\n\n<li>Getting a good night&#8217;s sleep is also essential, as <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC6042693\/\">chronic sleep loss<\/a> can lead to oxidative stress.&nbsp;<\/li>\n<\/ul>\n<\/li>\n<\/ol>\n\n\n\n<ol class=\"wp-block-list\" start=\"2\">\n<li><strong>Seek medical help to try to treat any underlying endogenous causes<\/strong>. For example, if you have a <a href=\"https:\/\/www.givelegacy.com\/resources\/varicocele-and-male-fertility\/\">varicocele<\/a> causing oxidative stress and affecting fertility, a varicocelectomy (varicocele repair surgery) can correct the problem.<\/li>\n<\/ol>\n\n\n\n<ol class=\"wp-block-list\" start=\"3\">\n<li><strong>Consider consumption of antioxidants<\/strong>. Look for reputable, high-quality vitamins like <a href=\"https:\/\/www.givelegacy.com\/supplements-coq10\/\">coenzyme Q10<\/a> (which has the strongest evidence and research studies behind it), vitamin C, or vitamin E. You can also increase antioxidants in your diet by eating foods such as blueberries, strawberries, raspberries, kale, spinach, beets, and beans.<\/li>\n<\/ol>\n\n\n\n<div style=\"height:50px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h2 class=\"wp-block-heading\">Other ways to improve fertility<\/h2>\n\n\n\n<p>Many general ways to help improve male fertility will also help reduce oxidative stress.
