An advance medical directive is a legal document that outlines your choices for health and end-of-life care, should you become unable to make these decisions for yourself.
If you are using the Department of Defense's Fertility Preservation pilot program to pay for sperm freezing, this program requires participants to have an advance medical directive on file specifying what should happen to their frozen sperm if they die. This is a program requirement set by the DoD, separate from anything Legacy handles directly.
If you're not part of the DoD program, an advance medical directive is not required. As part of our process, Legacy will ask you to complete a bequeathment form that specifies who or what should be granted ownership if your sperm in the event of your death.
